Overview:

The Patient Navigation Specialist handles referrals for community member requesting assistance with establishing with a Conway Regional Healthcare Provider. This position is responsible for the scheduling, sending, and receiving of medical records, and communications with all parties required to complete the community referral. This position completes data entry for Transitional Care Team for patients discharged from Conway Regional Health System. The Patient Navigation Specialist assist with scheduling follow-up appointments post discharge. The Patient Navigation Specialist must comply with Conway Regional Health System's policies and procedures

Qualifications:

• MINIMUM JOB QUALIFICATIONS:

  • Proof of high school or equivalent or higher education
  • Medical terminology course work or experience preferred
  • Previous computer experience
  • Excellent communication skills

  Other education and experience may be substituted (if applicable) for the required minimum job qualifications with the approval of the Chief Human Resources Officer.
