Job Description:
We are seeking a Physician, highly competent in the realm of healthcare with a specialized focus and interest in community healthcare. Our ideal candidate will have a track record in serving underserved populations such as the elderly, chronically ill, at-risk children, as well as individuals at the end of life.

Mandatory Requirements:
- Doctorate degree
- A minimum of 2 years' experience in Primary Care
- Possess valid Drug Enforcement Administration (DEA) and American Heart Association Basic Life Support (AHA BLS) certifications/licenses
- Active Controlled Dangerous Substances (CDS) License
- Board Certified in Family Medicine or Internal Medicine in NJ

Additionally Preferable:
- At least 1 year of experience in Senior Care and Geriatric/LTC
- National Provider Identification (NPI) Number is a plus

This full-time role requires a dedicated Physician to ensure the provision of top-notch care practices, focusing on symptom management and improving the quality of life for homebound patients as well as their families in Monmouth County, NJ. Emphasis is on candidates who have completed their licensure in New Jersey as a physician, and have accrued experience and schooling in primary care settings. Geriatric experience is highly recommended. Confirmation of board certification in either family practice or internal medicine is required along with current CDS and DEA licenses. - Offering direct medical services and counsel to primary care homebound patients within established productivity guidelines
- Attentive monitoring of patients\u2019 conditions and responsiveness to treatments administered
- Maintaining comprehensive patient records utilizing the Electronic Medical Record and complying with all reporting mechanisms
- Ensuring full compliance with New Jersey Department of Health and Senior Services Ambulatory Care Regulations for Licensure
- Acting as a representative of the agency for patients and their families.
- Participating in community outreach programmes when deemed suitable.

Shift duration:
- One weekend per month, 8:30a-4:30p, including one summer and one winter holiday.
