A health services organization in New York City is currently seeking a new Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW) to join their Assertive Community Treatment (ACT) team at one of their inpatient mental health facilities in Manhattan.
Responsibilities:
The Social Worker (LMSW / LCSW) will be responsible for:
- Providing the expected number of services to clients each month, majority of which are in the community or patient's homes
- Completing required documentation in a timely and high quality manner
- Working collaboratively with clients, their families, and community partners to assist clients in achieving their treatment and recovery goals
- Coordinating client care through close involvement with hospitals, substance use treatment facilities, etc.
- Screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations
- Participating in the development of treatment plans
- Counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning
- Providing client/patient advocacy and crisis intervention
- Developing, coordinating and evaluating services and support systems
Qualifications:
- 1+ year of relevant work experience
- Active NYS Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW)
- Training and/or experience in integrated Mental Health and Substance Abuse assessment & treatment, Motivational Interviewing techniques, and Stage-wise Treatment; or,
- Training and/or experience in Family Assessment & Intervention, Psychosocial-education and/or other Family Support services, including Cognitive Behavioral strategies
