**Essential Job Duties:**
* **Meet clients and families in their home setting**
* **Develop a plan of care including nursing and** support services that meet the client's needs
* **Determine what care giver team members are appropriate to meet the client's needs and manage that team ongoing**
* **Available to client and team on a daily basis to ensure services are provided timely and in full compliance with the clients stated desires.**
**Requirements:**
* **COVID vaccination required**
* **Licensed Registered Nurse. BSN preferred**
* **Prior home care experience REQUIRED**
* **Private pay experience preferred**
* **3+ years clinical experience**
* **Valid US Drivers License**
