*Minimum qualifications:*
* A Bachelor's Degree in Nursing and licensed to practice nursing in the State of Connecticut
* 7 years of experience in acute care, home care or long term care
* Certification in a nursing specialty
* Advanced knowledge and understanding of the healthcare delivery system and chronic care populations
* Strong organizational, communication, problem solving and decision making skills.
* Specialty certification in COPD preferred and/or a minimum of 5 years experience in management of COPD.
*Preferred qualifications:*
* *Masters of Science in Nursing*
* *Managed Care experience*
* *Case management experience*
