Job Description: The Home Health Field Nurse Supervisor will provide direct clinical supervision to field based clinicians, including RNs and LPNs, in support of the needs of the Agency. This position will validate ongoing clinical competence of field staff through co-visits, on-site supervisory visits, direct observation, and hands-on training and team meetings; communicate with physicians, therapists and the care team; assist in the achievement of patient outcomes (safety, cost, quality, satisfaction) by working collaboratively with interdepartmental teams; facilitates a professional climate which utilizes teaming and effective communication to meet patient care needs; monitor staffing adequacy and supplement by performing billable visits as needed.

Essential Duties: The Field Nurse Supervisor will supervise patient care and maintain awareness of complex conditions of patients through field visits; perform on-site visits to assess the care provided by LPNs under your supervision; Evaluate staffing needs for skilled patients and work with staffing coordinator to make adjustments to provide necessary staffing levels based on patient care needs. We estimate this position will be in the field approximately 35 percent of time.

  • Performs medication reconciliation weekly.
  • Perform On-site supervision of LPNs.
  • Educates and supports staff in meeting documentation timeframes. Disciplines staff as needed.
  • Ensures that skilled services are performed in compliance with regulatory requirements and practice standards.
  • Attends all nurses meetings.
  • Coordinates team's activities to ensure efficiency in delivery of patient care.
  • Perform program evaluations and prepare written reports as needed.
  • Makes initial nursing evaluation visit, as needed.
  • Coordinates physician appointments and outpatient procedures. Makes all arrangements to prepare patient before, during, and after appointment.
  • Makes joint home visit to train nurses on Trach / Vent Care to complete orientation skills checklist.
  • Approves time off and schedule changes for LPNs then makes recommendations to scheduler.
  • Provides skilled nursing care to select group of patients.
  • Skilled visit includes care plan coordination, collects labs and monitors all aspect of care. Makes doctors appointments and schedule transportation.
  • Answer field nurse questions
  • Assumes responsibility to become informed about purpose and process of appointment or procedure to be done
  • Orders supplies for all skilled patients
  • Asks physician or supervisor for information as needed
  • Performs assessments for Start of Care, Re-certs, Resumptions of care
  • Coordinates referrals for additional services; PT, OT, ST, MSW
  • Supervises HHA for all her / his skilled patients
  • Participates in the Quality Assurance/Quality Improvement Process
  • Prepares accurate, orderly and timely reports and documentation
  • Investigates patient / caregiver complaints with QA and Director of Clinical Services
  • Adheres to all practice standards as they apply to patient care
  • Responsible for ON CALL for skilled patients only. Approves time off for LPNs with schedulers input.
  • Other duties as assigned.

Required Knowledge, Skills and Abilities:

The Field Nursing Supervisor must be a caring, patient and a flexible teacher with excellent communication and time management abilities. The candidate must possess independent problem solving abilities, excellent clinical assessment skills and be computer literate. Additional requirements include:

  • Knowledge of PPS care delivery principles for maximizing patient outcomes.
  • Excellent writing and charting skills
  • Excellent patient assessment skills
  • Must be proficient computer skills including Microsoft Office and clinical applications/software
  • Must have highly developed interpersonal and critical thinking skills with the ability to prioritize needs rapidly
  • Requires the ability to coordinate information and activities, work under stress of deadlines and frequent interruptions, and to possess analytical problem solving skills.
  • Must possess diverse clinical knowledge and technology aptitude, including but not limited to:
  • Trach/Vent
  • G-Tubes
  • Blood Draws
  • Wound Care
  • IV therapy
  • Catheter maintenance
  • New diagnosis Education
  • Medication Management
  • Chronic illness management

Education and Experience:

  • Graduate of an accredited school of nursing; Bachelors of Nursing, preferred
  • Minimum five years nursing experience with at least one year in an acute care setting and at least one year Home Care.
  • Valid RN license in the District of Columbia.
  • Valid drivers license and one year clean driving record.
  • Experience with or familiarity with DC Medicaid, Medicare and OASIS documentation.
  • At least two years prior supervisory experience (clinical oversight, competency evaluations, etc) required

Physical and Environmental Condition:

Patient's home environments may involve risks or discomforts which require special safety precautions e.g. lifting, turning, transferring, assisting consumers when delivering nursing services and exposure to challenging physical and mental human behavior.

Office work environment involves everyday risks or discomforts that require normal safety precautions typical of such places as offices or meeting rooms, e.g., use of safe work place practices with office equipment, and/or avoidance of trips and falls.
