Job Description Summary:

This position administers routine physical therapy treatment as directed by a Physical Therapist. He/She monitors exercise programs, applies modalities and assists with discharge planning as directed by a Physical Therapist.

Minimum Qualifications:

Associate's Degree (Required)DL-HC - Driver's License only if in a Home Care department - Department of Motor VehiclesDepartment of Motor Vehicles, PTA - Physical Therapist Assistant - American Board of Physical Therapy SpecialtiesAmerican Board of Physical Therapy Specialties

Physical Therapist Assistant Program or graduate of accredited PTA program. Licensed by the State of Ohio. Participation in student clinic rotations. in modalities to prevent injuries. Continuing in area of interest relevant to work assignment. New graduates with excellent references and/or one year rehabilitation Experience.

For this position, in the Home Care department only, Minimum Qualifications include: Valid state driver's license, Minimum of eighteen (18) years old, Have an acceptable Motor Vehicle Report (MVR), Must provide proof of insurance, if applicable.
