Job Location Winnebago Public Health Department - Winnebago, NE **Job Purpose & Position Overview**

The purpose of this position is to promote diabetes prevention and health and wellness for patients living with diabetes through activities for children and adults in the community of Winnebago. This is a supervisory role in which the individual will mentor and manage the fitness staff. This individual will work in conjunction with the diabetes team to meet programmatic goals and foster improved health for patients living with diabetes through physical fitness. The Lead Fitness Specialist will function as a member of a multi-disciplinary team within the Whirling Thunder Wellness Programs and mentor to the fitness specialists.

**Specific Job Duties/Responsibilities:**

* Demonstrates the Winnebago Tribes core mission, vision, and values.

* Maintain skill level through on-going education and training on topics related to diabetes prevention, diabetes, behavioral interventions, teaching and learning or counseling skills workshops and in-service programs relevant to the diabetes prevention and diabetes program.

* Coordinate with the Director and the Diabetes Program Director to achieve outcomes for any grants, sub-grants, and contracts that relate to diabetes and prediabetes treatment, management and prevention.

* Perform frequent group classes for the seniors focused on diabetes prevention

* Analyze body composition data and interpret information for individuals as patient education, make recommendations and schedule follow-up body compositions.

* Develop and initiate community health promotion/diabetes prevention events as consistent with the Whirling Thunder Program annual goals and objectives.

* Function as liaison with local K-6 school children to foster participants and communication regarding all youth wellness activities.

* Responsible for data collecting regarding each activity/event, number of participants and able to furnish a monthly statistical and narrative report.

* Has access to confidential medical information, must maintain confidentiality regarding patient information.

* Performs frequent group educational sessions such as: in-service for local and area reservations, conference presentations and classroom presentations.

* Responsible for collecting appropriate data in accordance with SDPI Key Measures.

* Responsible for implementation of SDPI Best Practices that relate to physical activity in coordination with the Diabetes team for patients living with Diabetes.

* Ensure the maintenance and cleanliness of the wellness center areas.

**Required Qualifications/Skills:**

* Bachelors degree in education, recreation or related health field, or 2-3 years related work preferred.

* Must hold Personal Trainer Certification (CPT)

* Must be in excellent physical condition and be able to lead rigorous exercise sessions and be capable of performing strenuous duties.

* Must be willing to participate in required training opportunities.

* Must have CPR and First Aid.

* Must be able to work nights, weekends and holidays if necessary.

* Possess planning, development, and advanced organizational skills.

* Must be reliable, punctual and able to perform job duties independently.

* Advanced computer skills, knowledgeable in data base applications, word processing, and graphic layout.

* Be able to respond to emergency situations such as accidents, injuries, and disruptive behaviors of youth and adults.

* Must have valid drivers license and must be able to meet drivers eligibility guidelines.

* Be widely recognized as a positive health role model to the community: drug and alcohol free.

* Must complete Certified diabetes educator course.

**Supervisory Responsibilities:**

* Provide leadership manage fitness staff to ensure function of Program activities in accordance with grant goals.

* Develop and/or instruct group exercise classes specific for patients living with diabetes.

* Develop and/or instruct exercise programs specific for patients living with diabetes for individual participants.

* Development of specialized exercise programs for the purpose of promoting health and wellness for school age children and adults.

* Train and orientate any new wellness center staff to the facility, equipment and programmatic goals

* This is a supervisory position of 3-5 employees.

**Training Requirements:**

* Must be willing to pursue a training and education plan.

* Must attend all meetings and training courses as directed by Wellness Director.

**Physical Requirements:**

* Normal to heavy work which includes the ability to lift up over 50 lbs.

* Regularly required to run, walk, stand, and sit for extended periods of time and use hands and feet to operate various office and workout equipment.

* While performing daily duties, the employee is regularly required to talk and hear.

**Mental Requirements:**

* Reading, writing, calculating

* Above average social interaction skills

* Reasoning/Analysis

* Works with minimal to no supervision

**Language Requirements:**

* Must possess the ability to read, write, and speak the English language fluently.

* Must be able to continually and effectively employ professional verbal and written communications skills.

**Certificates, Licenses & registrations:**
