We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
The Team Nurse (RN/LPN) acts as patient advocate by assuming responsibility for coordinating patient's care.
Requirements
Licensure: Current NH RN or LPN license and BLS certification.
Experience: Community health experience preferred.
