Job Details

We are hiring a Social Worker to provide support in meeting the psychosocial needs of our residents, including their family members, in our Health Center (Skilled Nursing Facility) and Assisted Living Center. This social worker proactively manages and enhances residents abilities to achieve their highest level of independence, functioning, and satisfaction. This position also includes social services support to the Memory Care Unit s residents by providing psychosocial assessments that contribute to comprehensive service plans.

What You Will Do:

  • Identify, evaluate, and document the psychological needs of the residents and develop written plans to meet those needs either directly or through referral sources.
  • Advocate on the residents behalf to ensure the active promotion of resident rights.
  • Provide one-on-one counseling to residents to address individual concerns as required.
  • Utilize the ability of each resident to guide them to a self-determination lifestyle that provides a degree of satisfaction within the nursing and rehabilitation environment.
  • Safeguard appropriate and timely transition to other levels of care for the community s residents.
  • Make written and oral recommendations concerning the operation of the social services program.
  • Foster a climate of outstanding customer service. Play a key role to help ensure resident self-determination, dignity, and choices.
  • Participate in the Quality Assurance and Performance Improvement Process, evaluating and implementing recommendations from established committees.
  • Additional responsibilities will be outlined during the interview process.
