**In-home Care Attendant**
**MINIMUM QUALIFICATIONS**

* Must be 18 years of age or older.
* Must possess basic math, reading and writing skills.
* Must possess a valid social security number.
* Must be willing to submit to a criminal record check, child abuse clearance and FBI fingerprinting. Must PASS Criminal, Child Abuse and
* FBI fingerprinting clearances according to DPW regulations.
* Must demonstrate the capability to perform health maintenance activities required by consumers Person-Centered Service Plan or be willing to receive training in performance of health maintenance activities.
* Must have the required skills to perform personal assistance services specified in consumers Person-Centered Service Plan.
* Must support the Independent Living Philosophy.
* Must submit and pass TB test

**ESSENTIAL RESPONSIBILITIES**

* Provide essential personal assistant services such as bathing, dressing, bowl/bladder management, transfer assistance, meal preparation, light housekeeping and other tasks as outlined on consumers Person-centered Service Plan.
* Ability to lift, bend and perform other physical tasks as directed by the consumer or supervisor.
* Ability to travel when needed.
* Ability to work a varied schedule including weekends and evenings.
* Ability to be punctual and dependable.
* Ability to be respectful of all individuals for whom you work, both in the community and in the office setting.
