*Job Responsibilities:*
Under the supervision and direction of a licensed nurse, responsibilities of this position include, but are not limited to:
* Performs activities of daily living for residents such as bathing, oral care, grooming, dressing, toileting, feeding, walking, exercising and encouraging independence
* Observes patients' conditions, measuring and recording food and liquid intake and output and vital signs
* Transfers residents via wheelchairs and performs patient transfers in accordance with plan of care (i.e. sit to stand and hoyers)
* Passes medications, if able to provide documentation of completed medication training
Qualifications:
* previous experience in a long-term care facility preferred, but not required
* must be able to express self in written and oral communication and communicate effectively in care setting with residents, families and staff members
* ability to maintain confidentiality
* ability to work independently or with a team
* conducts oneself in a caring, sensitive and safe manner which promotes positive relations and rapport with residents and visitors
Educational Requirements: *Must hold a current Certified Nursing Aide certification in the state of Wisconsin*
