Summary:
The Care Manager R.N. reviews the admission and continued stays of all patients in the Case Management Program, the purpose of which is to assure high quality patient care and effective utilization of available health facilities and services. He/she is responsible for assessing the appropriateness of the level of care, diagnostic testing and clinical procedure performed; quality and risk issues; and documentation within the medical record. Information is abstracted from the patient chart and forwarded to Quality Improvement, Infection Control and other departments as necessary. He/she refers cases not meeting screening criteria to the appropriate physician advisor.
Experience/Education Requirements:
- Graduate of an approved school of nursing, BSN preferred.
- A minimum of 5 years medical/surgical experience
- Experience/certification in critical care nursing is preferred
- Previous experience with Case Management and Utilization Management is preferred
License or Certification Requirements:- Current Illinois State License as a registered nurse
- Current CPR certification with re-certification annually preferred
Responsibilities:
- Assesses the appropriateness of the level of care, diagnostic testing, clinical procedures performed; quality of care and risk issues; and documentation within the medical record; essentially progressing the care of the patient.
- Communicates effectively and collaboratively with all medical staff and other members of the interdisciplinary team.
- Completes care management functions including patient/family education.
- Discusses individual cases and follow-ups with the Attending Physician via expected daily rounding or through telephone communication.
- Documents progression of care daily in the EMR.
- Ensure the use of protocols and practice guidelines for inpatients as assigned.
- Follows through with established list of frequent readmission (5 patients) and reports back on their status.
- Identifies adverse patient care trends and follows through by notifying the appropriate staff, department, or Director.
- Maintains specified case loads for inpatients and progresses care for Outpatients Inhouse w/LOS list.
- Other duties as assigned.
- Participates in the discharge planning process within their caseload.
- Provides medical, nursing and ancillary staff with education on care management functions by attending staff meetings and other forums monthly.
- Refers reviewed cases to a Physician Advisor as needed.
- Satisfactorily complete all required continuing education hours regarding the care of acute stroke patients.
