Hybrid Remote/In-Person Full-Time Position in Castro Valley, CA and surrounding Counties (Non-exempt)

Our services include emergency shelter, transitional housing, rental subsidies, mental health services, and wrap around services. All our services are trauma-sensitive and client-centered and are provided at no cost to our clients. There are no barriers to participate in our programs.

About the Mobile Health Program
In 2023, Ruby s Place was awarded a grant from the CalOES to focus on domestic violence victims that may not have access to regular health care. The grant allows Ruby s Place to provide domestic violence victims/survivors along with their children access to COVID-19 testing, vaccines, and primary health care through mobile health/telehealth care visits throughout the 12 counties of the Greater Bay Area. The funding for this grant is through July 31, 2025.

What qualifications are required for this position?
You Are Also:
- A professional with 1-3 years experience in building collaborative public health networks that may include community based organizations, health clinics, faith-based organizations, advocacy organizations, local services providers, etc.
- Experienced with both online and in-person stakeholder facilitation, including the event design, execution and ongoing management.
- Experience designing equity-driven outreach materials in alignment with project campaign and brand standards.
- The State mandated 40-hour Domestic Violence and Human Trafficking counselor certification is required (this paid training can be scheduled after hire).
- Must possess a valid CA driver's license, proof of insurance, and be approved by Ruby's Place insurance as a driver in order to drive agency vehicles.
- Background check, fingerprinting (Live Scan), completion of all Community Care Licensing documents are required. All licensing requirements of Community Care Licensing must be met.
- Bilingual with proficiency in Spanish preferred.
What You Will Do
You are committed to removing client barriers to primary health care by identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health. You will work directly with clients and members of the mobile health team to understand client health needs and to support positive client health outcomes.
Responsibilities include
- Identify client needs and available community resources.
- Cultivate partnerships with community organizations to build capacity and expand informed care/services provided to women.
- Communicate with mobile health team members to facilitate client access to care.
- Serve as patient advocate and assist in identification and improvement of service delivery.
- Identify resources for clients to overcome barriers to care, such as transportation, child care, and health literacy.
- Accompany clients to providers as needed.
- Maintain strict confidentiality in accordance with agency policies and HIPAA requirements.
- Conduct a Community Health Needs Assessment with clients to identify gaps in women s health promotion, education, and healthcare access.
- Locate and help facilitate local classes on sexual and reproductive health and other women s health topics.
- Support access to health care by providing in-depth orientations to clients.
- Research and develop women's health promotion strategies.
- In coordination with the Data Team, Track outcomes, analyze data, and provide recommendations for program improvement
- Provide information and referrals (internal and external) for supportive services and public health providers using confidential, holistic, client-centered, trauma-sensitive practices.
- Fill out a functional timesheet by assigned due dates and in compliance with the time allocations.
- Actively participate in reflective supervision
- Create and maintain documentation of processes and procedures in relation to duties of the job
- Other duties as assigned by your supervisor
Working Conditions
This position is eligible for hybrid in person & remote work. Time spent on-site versus remote working will be discussed and agreed upon with your supervisor. You are expected to be available on-site as necessary given business needs. You must be able to stand for long periods of time and perform physical activities, including lifting up to 25 pounds. You will be subject to frequent interruptions and must be able to multitask and handle stress.
Local travel in the greater Bay Area is required.

The pay scale for the Community Mobile Health Advocate is $28.85 per hour. This is a full-time, (40 hour a week) non-exempt position. This is a hybrid position (flex work), with some days required in our Castro Valley Office.
