Position: Advanced Practice Provider -Nurse Practitioner/Physician Associate

Reports to: Medical Director of Iris Health Medical Group

Job Summary: The Advanced Practice Provider (Nurse Practitioner/ Physician Associate) for Iris Health Medical Group provides medical care for patients. Works under the general supervision of the Medical Director of Iris Health Medical Group. The role requires clinical excellence, organizational competency, the ability to handle multiple priorities, and outstanding interpersonal and communication skills. Candidates should have a strong desire to work as a member of the care team to meet the challenge of building a novel care model for a vulnerable patient population.

Responsibilities/ Essential Functions:

  • Provision of high quality and compassionate Primary Care Services for medically and socially complex, elderly, and homebound patients.
  • Practices within the scope of the California Board of Registered Nursing and/or Physician Associate practices and under the oversight of the Medical Director, and within organizational guidelines.
  • Coordinates assessment, treatment, and management of patients through consultation with the supervising physician to facilitate comprehensive patient care plans.
  • Diagnoses or treats acute health care problems, illnesses, infections, and injuries.
  • Analyzes and interprets patients histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Provides health care screenings for both preventative health and chronic conditions.
  • Counsels and educates patients regarding preventative health, chronic conditions, and disease prevention.
  • Prescribes appropriate medications (dosage and route) utilizing best practices.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
  • Documents in E.H.R. physical findings, formulates plan/prognosis based on patient's condition and follow-up with patient in an accurate, timely way.
  • Compliance with all state and federal laws and regulations, as they pertain to position including HIPAA, sexual harassment, scope of practice, OSHA, etc.

Has knowledge of and/or is willing to learn all phases of care transitions and resources available for patients.

Through telemedicine, conduct video consultations and message with patients, including assessing, screening, diagnosing, and prescribing medication for patients as necessary.

Visits patients in their place of residence for monitoring vital signs, performing the history and physical examinations, reconciling medications, advanced wound care, and developing care/treatment plans independently or in coordination with a supervising physician.

Coordinate and communicate with the care team involved with each patient.

Participate in other Iris Health programs as needed.

Skills and Abilities:

Excellent computer skills, preferably with Windows, including Microsoft Office Suite

Excellent telephone and communication skills

Excellent organizational, writing, and verbal skills.

Ability to work independently, set priorities, and work well under pressure.

Ability to maintain a high degree of confidentiality.

Ability to demonstrate superior professionalism when dealing with patients and colleagues. and vendors.

Ability to solve complex problems and troubleshoot care needs.

Comfortable conversing with patients and families about goals of care and coordinating with the Palliative Care team when appropriate.

Proficient in working independently and in a multi-disciplinary team environment.

Resolves routine administrative issues independently and in a timely manner.

Minimum Qualification:

  • Completion of certified Family Practice Nurse Practitioner or Physician s Assistant program.
  • Current RN/NP or PA license with corresponding State Board of California
  • Current active DEA registration or the ability to obtain upon hire.
  • Must successfully complete the provider credentialing process in accordance with National Accreditation requirements.
  • Current Basic Life Support certification required.
  • Experience as a Family Nurse Practitioner or Physician s Assistant in the area of primary care, or experience in assessing, diagnosing and treating a broad range of conditions, including medication management, under supervision of Provider.
  • Care and compassion in working with low income, indigent, as well as ability to work with other racially, culturally, ethnically, and financially diverse populations.
  • Ability to chart visits comprehensively and complete E&M coding in compliance with medical coding standards.
  • Experience with eClinicalWorks or other electronic health record systems.
