Role Description

This is a full-time remote role for a Registered Nurse, Case Manager. The Registered Nurse, Case Manager will be responsible for assessing and coordinating patient care, developing care plans, monitoring patient progress, and advocating for patient needs. They will also collaborate with other healthcare professionals, provide education and support to patients and their families, and ensure compliance with regulatory guidelines.

Qualifications

• Current Registered Nurse (RN) license
• Experience in case management and home healthcare
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and remotely
• Knowledge of healthcare regulations and guidelines
• Attention to detail and organizational skills
• Compassionate and patient-centered approach
• Bachelor's degree in Nursing or related field
• Certification in Case Management is a plus
