Our Community Health Team at Central Vermont Medical Center has an opening on their team for a Clinical Care Coordinator!

POSITION SUMMARY The Clinical Care Coordinator is responsible for the coordination and care of patients receiving Medication Assisted Therapy (MAT) for opioid addiction. Teaming with a Registered Nurse, the Clinical Care Coordinator the position provides health home services to a panel of 100 MAT patients in coordination with prescribing buprenorphine medical providers within the Central Vermont Health Service Area. This position as part of a high quality, embedded multidisciplinary support service that provides ongoing care planning, care coordination, and transitions of care support. Works closely with prescribing buprenorphine providers and members of the MAT Team to create and/or enhance effective and efficient services of the Vermont Hub & Spoke model. As a member of a care team you will be responsible for improving the care of your assigned patients across health, substance abuse and mental health treatment. Emphasis is placed on enhancing patient activation and empowerment for shared decision-making in treatment, healthy behaviors, and self-management of their own health, mental health, and substance abuse conditions. To be successful in this role the individuals applying for this position must have high levels of social and emotional intelligence with the ability to develop strong interpersonal relationships in both the therapeutic and professional domains.

BASIC KNOWLEDGE

Candidates must have experience with screening and assessment for common mental / substance use disorders with working knowledge of differential diagnosis and evidenced-based psychosocial treatments for common mental health disorders. You must have the ability to work well in partnerships with community partners, staff, and other healthcare professionals. Priority will be placed on finding a candidate with the ability to manage priorities, multiple tasks and workflow simultaneously, and organize work responsibilities effectively with attention to detail. Familiarity with brief, structured counseling techniques (e.g. motivational interviewing, behavior activation) preferred.

MINIMUM REQUIREMENTS

VT Licensed health care professional with mental health and/or addiction medicine experience (Licensed Clinical Social Worker, Licensed Independent Social Worker, Licensed Alcohol and Drug Counselor). Candidates with within 6 months of securing licensure will be considered. Frequent travel to multiple clinic locations is required.

EXPERIENCE

Successful candidate will have at least 5 years' experience in a mental health setting or substance treatment program. Prior experience in a primary care practice preferred.
