Judge Healthcare is urgently seeking a Social Worker for an opening with one of our biggest clients in Lowell, MA!
Summary:
As a passionate Licensed Social Worker at our facility in Massachusetts, your primary responsibility is to oversee the Social Services Department. You ll play a vital role in planning, organizing, implementing, evaluating, and directing social service programs. Your administrative authority ensures the effective execution of assigned duties.
Job Functions:
- Collaborate on planning, developing, and organizing social service programs.
- Participate in discharge planning, resident assessments, and social care plan development.
- Conduct interviews to gather residents social history.
- Refer residents and families to relevant community agencies when necessary.
- Provide consultation to staff and community partners regarding residents social and emotional needs.
- Interpret residents psychological and emotional needs for the care team.
- Obtain resources from community agencies to meet residents needs.
- Maintain confidentiality of residents protected health information.
- Contribute to policy and procedure development within the department.
- Regularly review policies and recommend updates.
- Complete required documentation, forms, and reports.
- Arrange transportation for residents as needed.
- Inform residents and families about financial assistance programs.
- Assist in annual job description reviews and updates.
- Make recommendations to the Director and delegate authority.
- Standardize methods to address residents social and practical needs.
Qualifications:
- Bachelor s degree in Social Work
- Minimum 1 year of supervised experience in healthcare
- Massachusetts licensure
