Job Summary

Under the general direction of the supervisor-patient placement, the RN-Transfer Coordinator is responsible for receiving and coordinating the referral of patients from both internal and external sources and facilitates assignment of patients based on clinical needs.

Education & Experience

Five (5) years of Nursing experience in Acute Care and/or Med Surg. Two (2) of these years in case management and/or utilization review preferred. BSN preferred.

Certifications, Licenses Or Registration Required: Valid RN license.

Knowledge, Skills & Abilities

Excellent interpersonal communication and negotiation skills. Demonstrates leadership skills. Strong analytical, data management and pc skills. Current working knowledge of utilization management, case management, and performance improvement. Understanding of pre-acute and post-acute venues of care and post-acute community resources. Strong organizational and time management skills as evidenced by capacity to prioritize multiple tasks and role components. Ability to work independently and exercise sound judgement in interactions with physicians, facilities, and patient families. Demonstrates commitment to organizational mission and values, process and behavioral expectations in all interactions and in performing all job duties. Performs duties in a manner to promote quality patient care and customer services/satisfaction, while promoting safety, cost efficiency, and commitment to the CQI process.

Responsibilities

  • Coordinates the patient activities of the transfer center; creatively manages resources through metrics to improve patient access care within the facility.
  • Collaborates with medical staff to assure timely access and safe, appropriate allocation of patients to clinical areas.
  • Identifies hospital policy, procedures and protocols which impact patient placement and access; makes recommendations for changes when appropriate.
  • Collaborates with the administration of an annual budget; monitors expenditures and implements measures to facilitate workflow, increase productivity, assure adherence to established budgetary guidelines, and maximize the utilization of space and resources.
  • Provides timely, regular information to medical, nursing and administrative staff related to issues of capacity and utilization; remains current on status of personnel admissions, staffing and bed availability.
  • Provides responsive feedback to referral sources and internal personnel facilitating patient access.
  • Applies approved clinical appropriateness criteria to monitor appropriateness of direct adult and pediatric admissions, and documents findings based on department standards. Identifies at-risk populations using approved screening tool and follows established reporting procedures. Refers cases and issues to care coordination physician advisor in compliance with department procedures with any quality issues identified. Refers high -risk or complex cases to unit based RN case manager.
