Job Summary:
Provides clinical data abstraction for national and internal registries. Based on data definitions and program requirements, reviews and synthesizes data from the electronic medical record/procedural records and enters into collection tool for interpretation. Collaborates with both the external registry and internal customers to understand data specifications and interpretation/risk adjustment methodologies. Collaborates with the clinical team to make improvements.

Job Description:Principle Responsibilities- Data Abstraction.
- Identifies appropriate charts for abstraction
- Reviews, evaluates and analyzes medical records.
- Locates and abstracts data from the medical record.
- Makes decisions and collects accurate data based on the specifications or project requirements.
- Enters information collected onto assigned data collection forms.
- Assures data entered into collection tool is accurate and complete.
- Reports errors or discrepancies in data extracts and tests new extracts for accuracy.
- Electronically submits data within specified time frame.
- Data Analysis.
- Reviews new specifications, identifying impact on current abstraction process.
- Participates in validation and inter-rater reliability exercises.
- Determines accurate data element interpretation.
- Monitors abstraction workload along with reporting deadlines.
- Data Dissemination.
- Collaborates with customers in order to understand data specifications and abstraction methodology as it pertains to medical record documentation.
- Assists in the development and maintenance of abstraction tools to reflect workflow and element changes.
- Mentors co-workers and new staff regarding workflow and measure education.
- Participates in quarterly validation process.
- Identifies opportunities for improvement and shares information with clinical team and leaders.
- Other duties as assigned.
Job Requirements- Associate's or Vocational degree in Nursing. Registered Nurse with at least 3 years of hospital clinical experience required or
- Bachelor's degree in Nursing. Registered Nurse with at least 3 years of hospital clinical experience preferred
- 3 to 7 years experience in a Hospital clinical environment, hospital coding environment, or other relevant experience required
- 3 to 7 years experience with Electronic Medical Record, Chart review, Microsoft office software, window environment, data entry preferred
- 3 to 7 years experience with a high comfort level for working with physicians, nurses, quality leaders and other key stakeholders and thinking to anticipate consequences and trends required
- 3 to 7 years Detail oriented, typing/data entry accuracy, strong organizational and time management skills, ability to be flexible and adapt to change, ability to prioritize workload required
- Licensed Registered Nurse - MN Board of Nursing required
