Position Summary

The Residential Habilitation Specialist who will be responsible for all aspects of Staff Action plans as per OPWWD and Medicaid Waiver regulations as well as Agency policy. The Residential Habilitation Specialist will create and/or monitor the Implementation of residential/day habilitation plans, ensure person centered planning and work with supervisors and direct care staff to ensure plans are effective and documentation is completed accurately and within regulatory guidelines.

Primary Duties & Essential Functions

  • To develop and monitor the Implementation of each Individual's Staff Action Plan and

Safeguards in the IRA program(s)/Day Habilitation program in accordance with regulatory

standards.

  • To train and make sure staff are knowledgeable in the procedures necessary to Implement the Life plan.
  • Implement, evaluate, revise and monitor Staff Action Plans and submit in a timely manner
  • Collaborate with treatment teams, family or guardians and Care Managers to ensure that all

Individuals' needs are addressed through the Staff Action plan and Safeguards.

  • Show consistency of specific valued outcomes identified by the treatment team as outlined In the Life Plan in Staff Action Plan and Safeguards.
  • Monitor Individual's progress in a weekly documentation check, monthly summary notes, and will update the plan minimally semiannually.
  • Update Staff Action Plans and Safeguards as needed In between Life Plan reviews to meet the needs to the Individuals or the requirements the Care Manager and agency.
  • Forward the Staff Action Plan and Safeguards to Care Manager within 60 days of the Life Plan review or the start of the Individual's residential habilitation service (whichever date Is first).
  • Facilitate the distribution of Staff Action Plan and Safeguards to Care Manager on behalf of the Individual within the timeline of the Life Plan review date.
  • Completes compliance audits of individuals clinical chart and ensure completion of clinical

charts.

  • Participate In committees assigned.
  • Attends trainings to ensure on going professional development.
  • Takes an active role in the intake process.
  • Other duties and responsibilities as assigned by supervisor or designee.

Required Education, Knowledge, And Skills

  • Bachelor's degree in human services or related field
  • Must have a least 2 years' experience in serving individuals with developmental disabilities
  • QIDP status
  • Must have intermediate skills In Microsoft word and excel
  • Demonstrate skills in writing

Abilities And Working Conditions

  • Must have a valid NYS driver's license
  • Must be able to perform restraints and maintain SCIP/Promote certification
  • Willingness to respond to the needs of a culturally diverse population
  • Ability to work out in the community {mobile program)
  • Ability to deal positively with changing priorities in a fast-paced environment.
  • Display a positive, professional and proactive attitude to accomplish quality work that is in with the agency's mission statement and sanctuary philosophy
  • Seek opinions, alternative viewpoints in decision making. Understand and respect the

expertise and experience of others, appreciate the different views, and the perspective and

limitations of other individuals

  • Ability to communicate effectively verbally and in writing effectively and professionally
  • Exhibits a professional, courteous demeanor with Internal and external constituents

Pay: $23.00 per hour
