Job Description

High Desert Medical Group is seeking a full time Health Care Assistant. This individual will be responsible for increasing access to Primary Care for our members and managing the post-referral coordination process. They will engage and encourage members to embrace wellness and prevention, including wellness visits and quality measures while adhering to an established treatment plan.

Summary of Duties:

  • Communicate with new members through various mediums, including telephone calls, emails, standard mail, and direct communication, when applicable.
  • Communicate with the patient, caregiver, specialist(s), and the assigned case manager(s) in effort to 'close the referral loop' and ensure thorough and quality care is provided to the member.
  • Facilitate specialty appointments and reschedule as necessary to meet each member's needs, including cancelling unnecessary appointments using established guidelines.
  • Retrieve pertinent outstanding data, such as post-hospital and urgent care follow up; including but not limited to prescribed medication along with other data that will aid in facilitating proper and timely primary care services.
  • Conduct and collect health assessments and questionnaires.
  • Refer to the Disease Management Program, Complex Case Management, and Social Work departments to ensure delivery of appropriate and efficient care coordination.
  • Determine basic needs, including but not limited to, social needs, transportation availability, and access to means of communication (i.e., telephone, email, or standard mail).
  • Follow and adhere to HDMG's standards of attendance policy, as it is pertinent to be available to assist our members during the post-referral process, ensuring that they are satisfied with the quality of care provided.
  • Perform other duties as assigned.


The pay range for this position at commencement of employment is expected to be between $37,960.00 and $52,665.60/year, non-exempt; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Details of participation in these benefit plans will be provided if an employee receives an offer of employment.

If hired, employee will be in an "at-will position" and the Company reserves the right to modify base salary (as well as any other discretionary payment or compensation program) at any time, including for reasons related to individual performance, Company or individual department/team performance, and market factors.

Requirements

  • High School diploma or equivalent.
  • Current Certified Medical Assistant (CMA) license preferred.
  • Bilingual in Spanish preferred.
  • Excellent written and verbal communication skills.
  • Ability to handle and prioritize tasks in order to meet all given deadlines and productivity goals.
  • Ability to handle matters of a confidential nature.
  • Ability to work in a multi-task, high-demand environment.
