Healthcare Recruiters seeks a full-time onsite RN Care Team Lead for our mental/behavioral health client s Crisis Respite Residences in Plattsburgh NY. SIGN-ON BONUS!


The RN Care Team Lead will assist the Crisis Respite Residences, both children and adult facilities, in ongoing triage and assessment of needs for the duration of stay at the crisis residence. RN Care Team Lead also assists residence staff in ongoing support provided to residents in order to develop, learn and apply the skills necessary to live more independently.


All shifts are stay awake shifts. The hours are broken into 7am-3pm, 3pm-11pm, and 11pm-7am. Shifts may include evenings, overnights, weekends, holidays, and on-call based on the program needs.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Triages referrals to determine if they meet admission criteria for crisis residence and provides alternative referrals when needed.
  • Assists in completing comprehensive psychosocial assessment that includes risk and health assessments upon admittance to the residence.
  • Identifies critical impairments that become the focus of service planning and develops and composes a clear service plan that includes strategies based on specific needs of each resident and presents plan to the resident, support system and care teams.
  • Provides supportive and educational group/individual/family counseling including training on medication monitoring, and management.
  • Coordinates services and communicates with resident's care team to include mental health and chemical dependency treatment providers, primary care providers, probation, parole, other providers such as school and social welfare providers as clinically relevant.
  • Provides unscheduled and or emergent services as needed for crisis clients at any time during scheduled shifts.


SKILLS AND QUALIFICATIONS:

  • Valid NYS license as a Licensed Registered Nurse.
  • Extensive experience (3+ years) providing direct service in mental health and/or crisis field preferred.
  • Knowledge of substance abuse issues including features of alcohol and various drug abuse signs and symptoms, intervention strategies, awareness of associated medical concerns preferred.
  • Broad clinical knowledge of psychiatric disorders, and psychiatric diagnosis.
  • Ability to conduct clinical interviews and assess mental status.
  • Ability to work within a multidisciplinary team until collaborate across disciplines and programs
  • Extensive knowledge with PC applications, Microsoft Office software and electronic medical records strongly preferred.
  • Valid NYS driver s license, acceptable to insurance carrier, for required local travel.
  • CPR certification required within 60 days of employment.
