Position: Residential Individual Support Specialist

Summary: As a Residential Individual Support Specialist at Block Institute, you will collaborate with the clinical team in each residence to develop comprehensive plans and services tailored to the individual needs of our residents. Your primary focus will be on ensuring adherence to person-centered practices and providing guidance to Direct Support Professionals to maximize independence. You will also oversee the management of individuals' history to ensure accurate documentation and compliance with regulations.

Essential Job Duties and Responsibilities:

  • Ensure current and accurate Life plans/ISPs for all individuals served.
  • Provide individual program planning, assessment, development, and implementation.
  • Create and update Residential Habilitation Plan/Staff Action Plan semi-annually or as necessary.
  • Ensure Residential Habilitation plan and habilitation services align with CQL Valued Outcomes.
  • Conduct I AM Assessments and Life Planning Team meetings, and maintain meeting minutes.
  • Train DSP staff on service implementation and ensure compliance with regulations.
  • Ensure billing documentation corresponds with services listed on the plan and submit billing weekly.
  • Follow Quality Assurance plan guidelines and write monthly progress notes.
  • Monitor goal-related progress and update goal plans as needed.
  • Attend team meetings for each individual on the caseload quarterly and review meeting minutes monthly.
  • Provide regular site visits and weekly updates to the Supervisor.
  • Ensure program participants' health, safety, and well-being.
  • Develop working relationships with other providers and assist in correspondence and meetings.
  • Maintain cleanliness and safety of program participants at all times.
  • Uphold confidentiality and report incidents following agency protocols.
  • Use positive approaches in handling difficult situations and serve as a role model for program participants and staff.
  • Adhere to agency policies and dress code.
  • Remain sensitive to cultural differences of participants and staff.
  • Pursue professional growth and development by attending trainings and workshops.
  • Represent Block Institute in a manner that promotes the agency's mission and values.
  • Undertake additional tasks as assigned by supervisor or management.
  • Adhere to agency-wide training initiatives and attendance policies.


Qualifications and Benefits

Qualifications:
  • Bachelor's degree in health, human services, or Special Education; Master's degree preferred.
  • Minimum three years' experience working with individuals with intellectual disabilities.
  • Supervisory and training experience preferred.
  • Working knowledge of NYS OPWDD regulations.
  • Proficiency in Microsoft Word, Excel, and Therap software.
  • Training in Personal Outcome Measures evaluation tool preferred.
