The SBH Registered Nurse works with the patients, families and health care team in the development of the plan of care. He/she carries out nursing responsibilities and acts as a liaison between school and School Based health center settings, which will contribute to the healthy growth and development of the individual child. #BMSSJ
* Works continuously to ensure students immunization compliance for school attendance.
* Identifies social concerns that interfere with health status of a child in the school setting. Is proactive in making recommendations following established procedures.
* Independently contacts primary care provider as appropriate to support the individual care of school age children. Always acts as a patient advocate.
