Job Details

Under direct supervision of the Director of Health Information Management and Care Transitions and as a part of a multidisciplinary team, the Clinical Social Worker will provide therapeutic services to patients, including assessments, treatment plans and discharge planning.

ESSENTIAL FUNCTIONS

1. Assess the patient s social and emotional factors to estimate the capabilities of the patient and the family to cope with daily living.

2. Responsible for immediate discharge planning and documentation for all behavioral health patients and others as applicable.

3. Complete psychosocial assessment within 24-48 hours upon patient s admission to the unit.

4. Assist the Psychiatrist, Physician and other unit staff members to understand significant social and emotional factors in relation to the patient s mental health needs.

5. Help the patient and family to understand, accept and follow medical recommendations.

6. Provide services to help restore the patient to the optimum social and health adjustment within the patient s capability.

7. Assist the patient and family with personal and environmental difficulties which predispose towards illness or hinder the patient in obtaining maximum benefits from medical care.

8. Assist the patient and family in the utilization of appropriate community resources such as family and community agencies to help support the patient with resuming life in the community or to live with the disability.

10. Collaborate with nursing staff to coordinate discharges.

11. Maintain a healthy and safe working environment for the patient s family and other members of the health team.

12. Confirm that referral sources are viable and available for nursing accessibility.

13. Document all progress notes properly and accurately on the healthcare facilities EMR in a timely manner.

14. Conduct daily chart audits to ensure compliance.

15. Maintain patient s confidentiality. 16. Participate in case reviews and conferences.

QUALIFICATIONS:

Must have a Master s degree, or be within 12 months of graduating from a Master s program: Masters of Social Work (MSW), Licensed Clinical Social Worker (LCSW), Licensed Social Worker (LSW), Licensed Professional Counseling (LPC), Licensed Clinical Professional Counseling (LCPC) Must have a current License in the State of Illinois

Must have at least 2 years of social work experience in an inpatient or milieu setting

Must have experience working with patient with mental illness

Demonstrated ability in social work, theories, practice and clinical expertise

Experience in the care of adolescent behavioral/mental health

Strong communication and interpersonal skills in dealing with diverse population and medical staff

Must have proficiency and ability to document patient finding in the electronic medical records
