KPG Home Health is seeking a skilled and compassionate Home Health Registered Nurse to join our dynamic team on a PRN basis. As a Home Health RN, you will be crucial in providing quality healthcare visits to patients in the comfort of their homes.
Responsibilities:
- Conduct patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patient progress, ensuring accurate and timely reporting.
- Educate patients and their families on proper healthcare procedures and disease management.
- Collaborate with physicians and other healthcare professionals to coordinate comprehensive patient care.
- Maintain accurate and up-to-date patient records.
Requirements:
- Current RN license in the state of Arizona.
- Minimum of 1 years of nursing experience in home health.
- BLS certification.
- Strong clinical and assessment skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a collaborative team.
- Reliable transportation and willingness to travel to patient homes.
