JOB SUMMARY: The MVH Licensed Clinical Social Worker (LCSW) supports MVH's initiatives toward clinical integration. 1) improving quality and patient satisfaction, 2) improving outcomes and the overall health of the patient, and 3) reducing the total cost of care. Social Worker will provide assessment, intervention, and advocacy as needed to patients with varying levels of mental health conditions. This position requires a high degree of flexibility, independence, and knowledge of the health care system. It is important to understand and have a working knowledge of community resources to maximize emotional, social, and physical well-being of the patients being served by our MVH Primary Care Practices. LCSW will coordinate intervention with PCPs and other care providers as indicated.

DUTIES AND RESPONSIBILITIES:
Licensed Clinical Social Worker


1. Completes initial screening of patients referred from primary care providers to determine disposition and needs

2. Employs a wide range of interventions, such as care coordination/case management, risk screening and behavior change, educational information, referrals, and safety planning

3. Offers solutions to road blocks while implementing programs.

4. Monitors high risk patients within each clinic and follows up appropriately with clinic staff

5. Ensures all deadlines are met during implementation stages

6. Revises care plans as indicated by the patient's response to treatment and provider direction.

7. Performs patient care responsibilities considering needs specific to the standard of care for patient's age

8. Is knowledgeable of medications and their correct administration based on age of the patient and their clinical condition

9. Formulates a teaching plan based on identified patient learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate

10. Is able to perform treatments and provide services and education to level of licensure

11. Advocates on behalf of patients and their families to gain access to services and resources

12. Provides provider support in complex cases

13. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as a liaison between administration, patients, physicians and other healthcare providers

14. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care

15. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as a liaison between administration, patients, physicians and other healthcare providers population served. Able to interpret data about the patient's status in order to identify each patient's age specific needs and provide care needed by the patient group.

16. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors

17. Communicates appropriately and clearly to manager, co-workers and physicians

18. Maintains a good working relationship both within the department and with other departments within our organization

19. Acts as a resource for other less experienced staff to include additional staff educational duties as assigned

20. Documentation meets current standards and policies

21. Coordinates and supervises patient care by coordinating with outside practices

22. Completes annual education requirements and keeps licensure current

23. Maintains patient confidentiality at all times. Reports to work on time and as scheduled, completes work within designated time

24. Represents the organization in a positive and professional manner

25. Complies with all organizational policies regarding ethical business practices

26. Maintain flexibility in a growing environment and be willing to grow, change and accept new duties as the scope of primary care practices changes with the needs of the community and hospital

27. Assists MVH with resources available for adults with behavioral health needs, including but not limited to: outpatient individual or group therapy, psychopharmacology, partial hospital programs and inpatient programs, and self-management tools 28. Effectively tracks and documents calls, outreach, case reviews, and outcomes of referrals in EMR system

Qualifications

QUALIFICATIONS:

Education/Certifications: Licensed Clinical Social Worker.

Experience: 6 months required experience, 2-5 yrs clinical experience preferred

Equipment/Technology: Experience with Word, PowerPoint, Excel, Microsoft Office, and various EMR systems.

Language/Communication: Must be patient, courteous, compassionate, and respectful with patients and their families. Effective written and oral skills. Ability to comprehend and follow detailed instructions. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, patients and their families, co-workers, physicians, and administrative staff. Speaks clearly and persuasively in positive or negative situations. Ability to listen well and get clarification.

Mathematical Skills: General mathematic skills.

Mental Capabilities: Can concentrate for long periods of time in order to complete assigned tasks with a better than average attention span in order to listen, read and remember verbal and written instructions. Can understand input from the supervisor and/or manager concerning errors and instructions. Ability to deal with problems involving several concrete variables in standardized situations. A clear understating of the importance of patient confidentiality and sensitivity regarding complex health problems. Ability to use critical thinking skills to assess patients and intervene as needed within scope of practice.

Interpersonal: Ability to work well with others as a team player. Organized, detail oriented, and the ability to manage to multi-task while maintaining a heavy load of complex patients. Maintain a neat and professional appearance.

Performance: Must be able to handle conflict issues with both patients and staff. Must follow established recommendations and guidelines. Must meet deadlines as needed.
