POSITION OVERVIEW


Responsible for planning and implementing activities for clients with a mental illness and a co-occurring substance abuse disorder; to assist them in their efforts to maintain or improve activities of daily living, current levels of interdependence, community integration and sobriety maintenance.


SPECIFIC RESPONSIBILITIES


- Provide training in substance abuse prevention and sobriety maintenance incorporating a 12-step focus and evidenced based practices for co-occurring disorders.

- Provide transportation to and from community activities and AA/NA meetings

- Develop monthly activity calendar identify community resources to support sobriety.

- Perform drug screens when necessary.

- Complete the required documentation, which includes, but not limited to individual treatment plans and progress notes.

- Organize and facilitate social and recreational activities appropriate for ACT clientele.

- Provide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the ITP; provide individual supportive counseling; assist in symptom management, ensuring immediate changes are made in the treatment plans as client s needs change. Educate and support client s families, and advocate for client rights and preferences.

- Conduct comprehensive assessment of all domains of a person s life.

- Provide on call crisis intervention when assigned covering nighttime and weekend hours.

- Become an expert of the ACT philosophy and follow and maintain high fidelity to the ACT Model of Care.


PRINCIPAL ACCOUNTABILITIES


Planning/implementation of social, recreational and training activities to support sobriety

Completion of required ITP goals and required documentation.

Substance abuse education

Coordination of 12-step community activities


EDUCATION, WORK EXPERIENCE AND JOB REQUIREMENTS


Education: Bachelor s Degree in Psychology, Human Services or Addiction Counseling preferred and/or one to three years equivalent work experience. Requires minimum ADT Trainee Status or CAC, CAC-AD



Job Requirements:


Certified or eligible for certification in addictions treatment. Must have a valid driver s license. Must have a clean driving record. Must have the ability to work independently with minimum supervision
