JOB SUMMARY

The Quality Coordinator role is part of the Pennant Quality Team and reports to the Director of Quality (DOQ). They will assist the Quality and Clinical Support Teams with monthly, quarterly, and yearly tasks as well as perform administrative duties. The Quality Coordinator will work together with quality and clinical leaders to plan, track, and execute processes that improve results.

DUTIES & RESPONSIBILITIES

  • Partner with department resources to support existing quality and clinical organization reports - includes monthly scorecards for clinical and quality outcomes.
  • Create and maintain comprehensive documentation, reports and records related to quality improvement activities.
  • Assist in preparation of quality improvement meetings and training sessions.
  • Support the implementation and monitoring of quality performance improvement projects.
  • Work with Quality Partners to collect data to identify areas for improvement.
  • Ensure CAHPS, HIS, and OASIS compliance through tracking, monitoring, and compiling of data.
  • Partner with additional resources to support new start-ups, partnerships, and acquisition's compliance within the Quality Reporting Program.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
  • Must possess, as a minimum, a high school diploma or its equivalent. Associate's Degree preferred.
  • Experience with Home Health and Hospice quality improvement concepts and practices preferred.
  • Extensive experience in Microsoft Office Suite Applications - Excel, Outlook, Teams, Power Point, Word.
  • Ability to collaborate effectively with individuals at all levels of the organization.
  • Strong commitment to maintaining confidentiality and handling sensitive information appropriately.
  • Excellent interpersonal skills and the ability to work well in a team environment.
  • Strong problem-solving and critical-thinking skills and ability to adapt to changing environment.
  • Excellent communicator.
  • High attention to detail. Responsible for many moving parts and projects.
  • Takes ownership in their results & processes.
  • Self-Driven. Doesn't rely heavily on permission or direction.
  • Professionalism in all aspects of interaction and communication at work.
