The Caseworker will provide onsite or remote services by completing assessments, exit services plans, provide support with travel coordination and arrangements to assist asylum seekers transitioning out of the city's supportive housing and resettlement services programs.

Duties and Responsibilities:

  • Complete assessments, exit services plans and provides support with travel coordination and arrangements to help asylum seekers transition out of the city's supportive housing and resettlement services.
  • Contact family, relatives, or others to coordinate travel.
  • Provide resettlement services outside of the city's supportive housing.
  • Complete referrals to community-based programs and organizations
  • Complete case notes, data, and information in the NY Health + Hospitals case management system.
  • Coordination with interpretation and translation services
  • Complete referrals to immigration legal services
  • Complete other tasks as necessary to facilitate delivery of necessary services to asylum seekers.
  • Performs other job-related duties as assigned.


Experience, Education, Knowledge, and skills:

  • Bachelor's degree or a minimum of one (1) year relevant experience working in social services with vulnerable populations conducting assessment and creating service plans.
  • Bilingual in Spanish and English (Speak, Read, Write and Translate) required.
  • Basic understanding of immigration laws, regulations, procedures, and benefits/entitlements.
  • Excellent verbal and written communication skills and ability to learn quickly and problem-solve under pressure and within prescribed timelines.
  • Excellent verbal, written and communication skills and the ability to work collaboratively with families, collaborating agencies, and community professionals.
  • Ability to organize, multitask and prioritize duties and responsibilities efficiently.
  • Proficient in utilizing Microsoft Excel, Power Point, Word, and Outlook.
  • Flexibility required due to potential shift changes based on operational needs.


Hourly Pay Range: $15.00 - $30.00
