Direct Care Home Health Services LLC is looking to add a new Home Health Aide to join our Washington DC team. The Home Health Aide will assist residents in their homes with all daily activities. These activities include everyday living, housekeeping, bathing and dressing, transportation, cooking, shopping, toileting, medication, mobility, and continence care.

The ideal person for this position has the experience, compassion, and a solid understanding of all aspects of patient home health care.

Major Duties:

To provide personal care services under the direction of our Registered Nurse (RN). The Home Health Aide is assigned to specific clients by the registered nurse or other appropriate professionals with in the company. The assignments are designed to enable the Aide to perform services for the clients that are necessary to promote good health and foster active and comfortable daily living.

Job Duties:

  • Performs personal care activities, including but not limited to: bathing, shampoo, skin care/nail care, oral hygiene, shaving and/or dressing.
  • Performs household service essential to health care at home, including but not limited to: meal preparation/feeding, laundry and light housekeeping.
  • Assist in the administration of medications that are ordinarily self-administration under the direction and supervision of the Registered Nurse (RN) (per state nurse practice law and agency policy)
  • Reports any observed changes in the clients condition.
  • Document all care provided on timesheets, complete the forms required for the clients' records.
  • Promote personal and client safety, by observing environmental and practicing infection control by following agency guidelines and reporting unsafe situations to the supervisor/case manager.
  • Demonstration of safe practice in the use of equipment. DO NOT USE equipment until orientation has been provided. Notify supervisor of educational needs.
  • Communicate effectively with all members of the interdisciplinary team through verbal report, participation in staff meeting and team.
  • Maintains confidentiality in aspects of the job.
  • Participates in required in-service programs to meet compliance requirements. Moreover, identifies learning needs to agency supervisor.
  • Performs other related duties and responsibilities as deemed necessary.


Qualifications:

  • Successful completion of a formal certification training program and/or written skill test and competency evaluation.
  • Must be at least 18 years of age.
  • Minimum one (1) year work experience in a supervised setting preferably health care facility.
  • Demonstrated ability to read, write and follow written Plan of Care.
  • Good interpersonal skills
  • Government issued picture ID

Required Knowledge, Skills and Abilities:

  • Ability to gain thorough knowledge of Direct Care Home Health Services LLC policies and procedures;
  • Ability to act as a representative of Direct Care Home Health Services to the public;
  • Thorough knowledge of program practices and procedures;
  • Ability to maintain effective work relationships with associates, subordinates, representatives from other agencies, and the general public
  • Ability to effectively communicate with individuals from diverse backgrounds;
  • Ability to use sound judgment when making decisions
  • Ability to pass a criminal background check

Physical and Environmental Conditions:

Patient's home environments may involve risks or discomforts which require special safety precautions e.g. lifting, turning, transferring, assisting consumers when delivering nursing services and exposure to challenging physical and mental human behavior.
