About The Job:The Social Services Technician collaborates with other members of the multidisciplinary team in caring for the psychosocial, financial, emotional, environmental factors as they relate to patient health needs; acts in concert with the social services and other acute population care management colleagues, nursing, medical, and other support staff in providing assistance and supportive measures to patients and families for care transitions, care coordination and care management - specifically focused on connecting patients with community resources and helping patients to be successful in removing socio-economical barriers; participates in ongoing continuous quality improvement processes to improve patient care.
What we are looking for:Education:
Bachelor of Science or Arts degree with a major in social services, psychology, sociology, human relations, health related field required.
Experience:
Previous experience working in connecting individuals to community resources is required.
Special Skills, Competencies, and Aptitudes:
Knowledge/skills with data systems, continuous quality improvement (CQI), community healthcare resources.
Ability to work independently in analyzing data, exercising judgment to arrive at conclusions.
Ability to establish and maintain working relationships.
Assertiveness and critical thinking.
Ability to communicate effectively and tactfully both verbally and in writing.
Skill in exercising a high degree of initiative, judgment, discretion, and decision-making.

Must be able to read, write, and/or speak English.
Must provide proof of
Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.
Completes
Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
