Harlem Children s Zone (HCZ) a world-renowned education and poverty-fighting organization based in New York seeks an enthusiastic, dedicated, and mission-aligned Health and Wellness Specialist for the Baby College GRADS program.

The Health and Wellness Specialist will bring a passion for the mission of Harlem Children s Zone: break the cycle of intergenerational poverty with comprehensive, on-the-ground programming that builds up opportunities for children and families to thrive in school, work, and life.

The Health and Wellness Specialist will join the Baby College GRADS program. The G.R.A.D.S. Early Head Start is a home-based program designed to promote the physical, social, emotional and cognitive development of young children and families from pregnancy to 3 years of age. The program runs for a full year and includes weekly home visits and bi-weekly socializations. The goal is to partner with families in strengthening their child s school readiness skills and well-being in accordance with Head Start Performance Standards to ensure children are happy, healthy and ready to learn.

Minimum Qualifications

  • Bachelor s degree in Early Childhood Development/Education or related subject area
Who you are
Bachelor s degree in Early Childhood Development/Education or related subject area; Bachelor's degree with CDA (Childhood Development Associate) preferred
Familiarity with the Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) program of the NYS Medicaid agency
Knowledge of the Centers for Disease Control and Prevention s immunization recommendations, and any additional
recommendations from the local Health Services Advisory Committee that are based on prevalent community health problems
Excellent communication skills; bi-lingual (Spanish and/or French) applicants are encouraged to apply
Familiar with the Harlem community and resources available in the community

What you'll do
Obtain determinations from health care and oral health care professionals as to whether or not the child is up-to-date on a
schedule of age appropriate preventive and primary medical and oral health care, based on: the well-child visits and dental
periodicity schedules as prescribed by
Assist parents with making arrangements to bring the child up-to-date as quickly as possible; and, if necessary, directly facilitate
provision of health services to bring the child up-to-date with parent consent
obtain evidence-based vision and hearing screenings within 45 calendar days after the child first attends the program or receives a
home visit
If a program operates for 90 days or less, it has 30 days from the date the child first attends the program to satisfy paragraphs (b)(1)
and (2) of this section
Identify each child s nutritional health needs, taking into account available health information, including the child s health records,
and family and staff concerns, including special dietary requirements, food allergies, and community nutrition issues as identified
through the community assessment or by the Health Services Advisory Committee
Provide ongoing care by assisting parents to continue to follow recommended schedules of well-child and oral health care
Conduct periodic observations and utilize appropriate strategies for program staff and parents to identify any new or recurring
developmental, medical, oral, or mental health concerns
Facilitate and monitor necessary oral health preventive care, treatment and follow-up, including topical fluoride treatments and
other necessary preventive measures, and further oral health treatment as recommended by the oral health professional.

Schedule
  • Monday-Friday- Must be available to work some Saturdays and some evenings

The annual salary for this position is $50,000, with a comprehensive benefits package including employer-covered health insurance.
