We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our hospice care team. The ideal candidate will provide end-of-life care, ensuring patient comfort and supporting family members during this sensitive time.
Key Responsibilities:
- Monitor patient health and administer medications as prescribed.
- Provide basic patient care, including hygiene assistance, ambulation, and feeding.
- Collaborate with RNs and doctors to implement comprehensive care plans.
- Educate and support patients' families on care techniques and end-of-life preparation.
- Document patient care services and comply with health and legal standards.
Qualifications:
- Valid LPN license with experience in hospice or palliative care preferred.
- Strong communication, empathy, and teamwork skills.
- Ability to work in a high-emotion environment, providing emotional support to patients and families.
