Sigma Healthcare staffing is looking for an experienced Physical Therapist to work for our contracted facility.
Shift Information: Mon-Fri
Responsibilities:
- Assess a person's condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.
- Collaborate with interdisciplinary team and colleagues to ensure a person's needs are met.
- Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.
- Utilize a variety of therapeutic modalities to maximize care provided to clients
Requirements: - Vermont PT licensure
- Home Health experience and OASIS experience a must as this candidate will case manage
- McKesson/Netsmart EMR experience strongly preferred
- New Grad are okay.
- SNF Experience preferred
- Covid test 72 hours prior to start date with results issued 48 hours prior to start date
