Bear Mountain Healthcare is seeking a full time (40hr/day shift) Director of Social Services for our skilled nursing facility, Mattapan Health and Rehabilitation Center, in Mattapan, MA.

The Director of Social Services is responsible for initiating and maintaining a comprehensive social services program on throughout the center. Social Services is responsible for identifying and providing for each resident s social, emotional and psychological needs. The Director will assist in the development of a safe discharge plan. This position also requires the timely completion of MDS assessments, 48-hour meetings, initial, quarterly, and annual assessments.
Long-term care experience is preferred; however, we are willing to train the right candidate!

Requirements:
LTC/Nursing Home Experience preferred
Licensed Required
