The RN-PCC (Registered Nurse - Patient Care Coordinator) applies the nursing process across all areas of practice, managing and providing healthcare and education to patients and their families during hospitalization.Responsibilities: Patient Care Coordination: Coordinates the patient care team to deliver comprehensive and compassionate care. Applies the nursing process to assess, implement, coordinate, delegate, and evaluate nursing care for patients. Prioritizes patient care needs based on acuity and provides care accordingly. Clinical Expertise: Performs specialized nursing care procedures as outlined in department/unit-specific competency profiles. Stays current with advancements in nursing practice and incorporates evidence-based interventions into patient care. Communication and Documentation: Coordinates patient encounters, admissions, transfers, and discharges efficiently. Maintains accurate and detailed documentation of patient assessments, interventions, and outcomes. Leadership and Team Development: Provides leadership to the patient care team, fostering a collaborative and positive work environment. Actively contributes to the professional development of team members. Healthcare Education: Educates patients and their families on health-related topics, treatment plans, and post-discharge care. Performance Improvement: Identifies opportunities for improvement in patient care and organizational processes. Participates in performance improvement activities to enhance the quality of care provided. Research and Professional Development: Actively participates in medical and nursing research activities. Engages in ongoing professional development to stay abreast of current trends and best practices in critical care nursing.
