Social Work Supervisor (Brooklyn-Housing)

Position Summary: To provide Selfhelp Active Services for Aging Model (SHASAM) services and supports appropriate for different stages in the aging process, made available to residents, if and when chosen and requested, throughout their tenancy in Selfhelp Housing. Meet client needs for supporting housing stability and reducing social isolation through an array of case management services.

Principal Responsibilities:

  • Supervise social workers and community workers in daily operations of case placement, - supervision, coordination, and/or client relations, and problem solve/resolve emergent issues.
  • Assist staff members through regularly scheduled individual conference in analyzing case problems and improving their diagnostic and helping skills.
  • Maintain established agency policies and procedures, objectives, quality assurance program and safety standards.
  • Consult with and keep department director informed of activities, needs, and problems related to demands.
  • Assure that staff adhere to mandates established by contract and that quality service as well as case coordination services are provided.
  • Assist in assessing the case worker's skills and identify specific training needs for staff.
  • Evaluate performance of staff periodically and reports to the director.
  • Supervise and direct case workers in developing written care plans and comprehensive assessment forms.
  • Attend case conferences with social work staff from other departments and agencies.
  • Attend community based.
  • Attend and actively participate in staff meetings.
  • Review case records to ensure that services are provided according to the assessed care plan.
  • Assist in resolving difficult client issues.
  • Help client and family through individual or group conferences to understand, accept, and follow care plan recommendations.
  • Participate in planning for improving services by interpreting social factors pertinent to development of program.
  • Provide coverage for workers who are out of office, sick or on vacation.
  • Substitute for Director in his/her absence.
  • Perform other duties as requested or assigned.
  • Adhere to organization's standards by completing annual mandatory trainings in a timely manner on topics including - but not limited to - Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA and any other training the organization may deem necessary in regards to regulatory compliance or good business practices.
  • Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
  • Adheres to the organization's policy in regards to absenteeism and appearance and health and safety standards.
Salary Range : 55,000 - 60,000 per year commensurate with experience

Job Competencies & Minimum Qualifications:
  • MSW required; LMSW preferred.
  • Two years human services experience.
  • One year supervisory experience.
  • Valid driver's license and car registration.
  • Working knowledge of Microsoft Office Suite and other technology.
  • Able to work independently.
  • Excellent verbal and written skills.
  • Excellent customer service skills.
Working Conditions/Physical Demand:
  • Must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds
  • Occasionally exposure to toxic or caustic substances and outdoor weather conditions; risk of exposure to pathogens and unsanitary conditions in client's homes.
