KEY FUNCTIONS & RESPONSIBILITIES:
* Provide education on preventive care, health promotion, and disease management to individuals and families within the target population.
* Assist individuals in navigating the healthcare system, scheduling appointments, and accessing necessary resources and services.
* Support the clinical team by assisting in screening, assessing, and completing other health-related tasks as needed.
* Establish trusting relationships with patients and their families and provide general support and encouragement.
* Conduct intake interviews with patients, including enrolling patients into, and/or referring them to, various health, assistance, and social service programs.
* Follow-up with patients, by phone, home visits, and visits to other settings where patients can be found.
* Assist patients with completing applications and registration forms.
* Conduct and verify eligibility determination, enrollment, and follow-up with uninsured patients.
* Help patients connect with transportation resources and give appointment reminders in special circumstances. Transporting patients is strictly prohibited.
* Be knowledgeable about community resources appropriate to the needs of patients/families.
* Act as a patient advocate and liaison between the patient/family and community service agencies (i.e., schools, Department of Human Services, Resettlement Agencies, hospitals, support groups, etc.).
* Record patient care management information in the EMR (training provided) and other software no later than 24 hours after patient contact.
* Attend regular staff meetings, training, and other meetings as requested.
* This position may require travel between CCI sites to provide additional coverage when needed.
* Other duties as assigned.
MINIMUM REQUIREMENTS:
* High School graduate, or equivalent
* Current CHW certification from an accredited program, highly desired.
* Current BLS/CPR certification required.
* Previous experience as a Community Health Worker, preferably working with diverse populations.
* Cultural competence and ability to work effectively with individuals from various cultural backgrounds.
* Excellent communication and interpersonal skills.
* Proficiency in languages commonly spoken within the refugee population (i.e., Pashtu, Dari, Haitian-Creole, Arabic, and Dinka) is required.
* Familiarity with the local healthcare system and resources.
* Ability to work flexible hours, including evenings and weekends, to accommodate the community's needs.
