Services Director
Skokie Meadows is currently seeking the services of a compassionate social services director for the geriatric population to join our team.
Social Services Director Essential Job Functions:
Identifies the social and emotional needs of individual residents.
Assists in arranging services to meet the needs of residents.
Participates in resident care planning.
Ensures the social and emotional needs or reach resident are met.
Provides advocacy between the resident, family, facility staff and the community.
Education and Experience Requirements:
MSW, LCSW, or LSW preferred
Previous experience in long-term care facility is preferred
Working knowledge of Medicare and Medicaid is preferred
Experience in creating quality assurance/quality improvement initiatives
Strong communication and multi-tasking skills
Compassionate and empathetic personality
