Summary:

Providespsychosocial services to the residents at their assigned facilities and workswith the facility Social Service Staff and Clinical Staff within their regionto ensure daily resident care is implemented per company and regulatoryguidelines. Assists with education, orientation and survey preparation/management for the region under the guidance and direction of the RegionalDirector of Social Services.Qualifications:Education :BSWor MSW

Licenses/Certification:FullLicensing as required by the state of employment

Experience:5years of experience in Social Services in a long term care environment

Essential Functions:Provides education, training and support to appropriatefacility staff to facilitate improved resident care and outcomes associatedwith their essential function/duties listed belowPrepare facilities for issues and concerns duringsurvey processProvides direct psychosocial intervention.Performs resident assessments atadmission, upon condition change and/or annually as needed.Creates, reviews and updatescare plan and progress notes as needed.Provides direct psychosocialintervention as needed.Assists with or completes thePASARR Process according to state and federal regulations.Assists resident's families incoping with skilled nursing placement, physical illness and disabilities of theresident, and the grieving process.Works with the facility SocialService Department in assisting patient, family and other team members to plandischarge.Conducts in-service programs toeducate staff regarding psychosocial issues and patient rights as needed.Stays abreast of all currentstate regulatory statues and updates that may impact essential functionsConducts job responsibilities in accordancewith the standards set out in the Company s Code of Business Conduct, itspolicies and procedures, the Corporate Compliance Agreement, applicable federaland state laws, and applicable professional standards .Performs other tasks asassigned.

Knowledge/Skills/Abilities:Ability to workcooperatively as a member of a team.Ability tocommunicate effectively with residents and their family members, and at all levels of the organization.Ability tomaintain confidentiality.Knowledge ofpsychosocial practices applicable to a long term care environment.Skilled in directing and motivating the workforce.
