Intake Coordinator - Home Health


We are seeking an experienced Intake Coordinator to manage the intake process for our home health agency. The Intake Coordinator will be responsible for receiving referrals, completing patient assessments, verifying insurance eligibility, and coordinating the start of care.


Responsibilities:

- Receive and review referrals from physicians, case managers, and other sources

- Verify insurance benefits and obtain authorizations

- Coordinate with clinical staff to initiate home health services

- Maintain accurate and up-to-date patient records

- Serve as the main point of contact during the intake process


Qualifications:

- Experience as an Intake Coordinator in a home health setting

- Excellent communication and interpersonal skills

- Ability to multitask and manage high-volume workloads

- Proficiency with electronic medical records software
