About Us: Connect Search is working with a local non-profit whio is dedicated to making a meaningful impact in our community by providing essential support and resources to those in need. As we continue to expand our programs and services, we are seeking a dedicated Case Manager
Key Responsibilities:
- Screen participants with Peer Specialist and Liaison.
- Complete intake paperwork and enter data into HMIS.
- Develop client-driven case plans, monitor progress, and empower participants.
- Refer to community resources, assist with budgeting and transitional housing.
- Foster communication and resolve conflicts; collaborate with service providers.
- Stay updated on assistance programs; remain calm in stressful situations.
Qualifications:
- Bachelor's degree in Human Services or related
- Proven experience in working within the case management field
- Strong communication and interpersonal skills.
- Ability to multitask and prioritize tasks in a fast-paced environment.
- Excellent organizational skills and attention to detail.
