• Support the health education program
• Participate in community health fairs
• Perform telephonic atrisk health education based on participants completed health risk assessment and health/lifestyle priorities

• Help meet community health needs
• Maintain all member health education materials
• Implement branded health promotion programs
• Assess health and wellness of the intended community
• Ensure all health education materials provided to members
• Integrate health education materials and information in health center operations and patient visits
• Coordinate, facilitate and teach community health programs
• Create community awareness regarding the importance of preventative health practices to improve health status and quality of life through fitness activities and nutrition-health-wellness promotion
• Develop and implement health education and disease management materials
• Develop health education materials as well as plans for program promotion
• Maintain approved health education materials at assigned sites
• Ensure that health education to support members' needs
• Needing behavioral health treatment through education and awareness activities
• Assess community need and ensure health education outreach activities target identified issues
• Attend health fairs and community events and festivals
