Pay Range: $26 - $31/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Note-Location: Arkansas CST Time Member Facing once in a Month.

Key Responsibilities:
- Member outreach; Health coaching members into healthy habits and gap closure; assist with membership attribution; potential provider engagement onsite with provider gap closure; assist with solving for SDOH concerns; provide resources to our members to improve health outcomes.
- 32 calls/day/160/wk; Reach rate of 20% of members called (6 members/day) to educate on preventative measures and assist members with scheduling Annual Wellness Visits.
Key Requirements and Technology Experience:
- LPN
- Health Coach
- Medical Office Assistant
- MSW
- Clinical experience working in a physician s office.
- Previous clinical outbound call outreach; motivational interviewing.
- HEDIS/Stars experience.
- Project Management.
- Case Management/Utilization Management.
- Population Health.
- Bachelor s degree in healthcare, Public Health, Nursing, psychology, Social Work, Health Administration, or related health field or equivalent work experience.
- Required Certifications: Preferrred: LPN, MSW, certified Health coach.
