Licensed Practical Nurse | Community Hospice LPN

  • Shift/Hours: Monday - Friday, First Shift, 9:00 AM-5:00 PM
  • Location: Manhattan/ Brooklyn, NY (1 position)
Experience
  • Licensed by the state as a Licensed Practical Nurse under Article 139 of the New York State Education Law.
  • Previous experience in hospice, home health, geriatrics, or general medical/surgical nursing.
  • Demonstrates a commitment to the Hospice philosophy of care.
  • Knowledge of Computer operation and computer-generated clinical records experience is preferred.
  • Must have a reliable automobile, valid driver's license, and the minimum state-required liability automobile insurance, unless ALL assigned patients can be served by using public transportation.

Licensed Practical Nurse | Community Hospice LPN Position Summary

Under the direct supervision of a Registered Nurse, provides routine patient
visits, continuous care services, in-home respite care services, inpatient care
services, and personal care for the patient. Supports the Primary Registered
Nurse, Provider Relations staff, and Quality Improvement activities of the
organization. Under Hospice state regulations, LPNs are not permitted to
implement initial nursing assessments or change the Hospice plan of care.

Licensed Practical Nurse | Community Hospice LPN Responsibilities Include
  • Delivers nursing services by Hospice policies. Follows the interdisciplinary Plan of Care. Is alert to the patient's needs and concerns and teaches appropriate personal-care techniques to patients and family members.
  • Provides direct personal care including bathing, dressing, grooming, caring for hair, nail care, oral hygiene, skin care, and foot care.
  • Assists the attending physician or registered nurse in performing specialized procedures as an extension of therapeutic services.
  • Prepares and utilizes supplies and equipment for treatments that require sterile or aseptic technique by the Plan of Care.
  • Assists patients with immobility including; transfers, ambulation, bed positioning, range of motion exercises, and safety utilization of all necessary equipment.
  • Assists and educates patient/family on patient's environmental needs. Assists patients in the use of bedpans, urinals, bathroom or bedside commodes, routine personal care, colostomy care, and enema administration according to the Plan of Care.
  • Assists patients with meals, if necessary.
  • Performs household services essential to maintaining a safe, clean, and comfortable environment.
  • Administers medications as prescribed.
  • Observe patients when giving care and report changes in the patient's condition or needs to the Hospice Nurse Coordinator/Primary RN.
  • Completes appropriate clinical documentation on time.
  • Participates in ongoing education programs.
  • Follows the Hospice of New York policies of confidentiality in compliance with HIPAA Regulations.
  • Demonstrates a compassionate attitude toward patients and families that is consistent with the Hospice philosophy and mission.
  • Attends patient care conferences as requested by the Hospice Nurse Coordinator.
  • Performs other related duties and special projects as assigned.
