Job Summary:
Department: Behavioral Health Services
Shift Timing: 08:00 - 16:30 . 3 days 8 hour shift.
Per Week: 24 Hours
Social worker LCSW, needed for part time hours= 24 hours a week, 3 eight-hour shifts (8-4:30p) workdays are flexible.
Might have some weekends, but not necessarily.
Previous behavioral health experience and discharge planning experience for Behavorial health patients is a requirement.
Roles & Responsibilities
Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
Assess and coordinate patient's discharge planning needs with members of the healthcare team.
