QUALIFICATIONS

Education & Experience

  • Currently enrolled in the equivalent of a Master's level degree programs working towards Associate Clinical Social Work, Associate Marriage and Family Therapist, and/or Associate Professional Clinical Counselor
  • Currently enrolled in the equivalent of a Master's Level Intern or practicum course for Marriage and Family Therapy, Clinical Counseling, or Social Work Practicum course
ESSENTIAL JOB FUNCTIONS

Position Specific
  • Provides comprehensive, strength-based, culturally competent, flexible, effective, quality mental health services, including psychotherapeutic interventions, to eligible at-risk children, youth, and families.
  • Provides services to assigned children, youth, and families in the home, office, school, or other community based settings
  • May coordinate and facilitate initial intake meeting with consumers meeting the agency's and County timelines and expectations
  • May be responsible for assessment, development, and implementation of core assessment documents and treatment plans
  • Provides Intensive Home Based Services (IHBS) and/or individual rehabilitation to youth as identified in youth's treatment plan
  • Provides collateral support to caregivers and significant support persons as identified in youth's treatment plan
  • Provides other Intensive Care Coordination (ICC) and/or case management functions as necessary, securing formal resources, and keeping team members informed of progress and challenges presented.
  • Participates in Child Family Team (CFT), multi-disciplinary team, hospital, crisis, psychiatric, court, school, and provider meetings as scheduled
  • Able to provide or arrange for safe transportation for youth and families to services as needed
  • May develops and co-facilitate rehabilitative and/or skill building groups
  • Develops and prepares updates to individualized safety plans in coordination with youth's child family team, maximizing the use of natural supports and community based resources
  • Provides and documents all Medi-Cal eligible billable and non-billable services, including but not limited to assessment, plan development, individual therapy, group session, group therapy, collateral, rehabilitation, In Home Based Services (IHBS) case management, Intensive Care Coordination (ICC) and crisis intervention within required timelines
  • Participates in the on-call rotation
  • Attends all staff meetings, clinical supervision, and consultation meetings as scheduled
  • Participates in utilization review process as assigned
  • Responsible for meeting all outcomes as documented in Employee Development Plan
  • Works collaboratively with all internal programs, departments, and staff
  • Maintains positive relationships with consumers and colleagues

  • Model and communicate appropriate positive attitudes toward the agency's Mission, Vision, and Values
  • Work collaboratively with all agency programs and staff to provide support as needed
  • Utilize and maintain calendar with all work-related details in order to manage time effectively and share calendar information with coworkers
  • Participate in on-going training to expand and develop professional skills
  • Perform other duties as necessary for the agency, as assigned


Employment At-Will
Employment at the Agency is terminable at-will, which means that employment may be
terminated at any time, without cause or reason, by either the employee or the Agency. In
addition the Agency may also demote, layoff, transfer or reassign employees at any time at its
sole discretion without cause or reason.
