The Registered Nurse Case Manager:
- Perform psychosocial assessments and discharge planning with medical input
- Facilitate communication between the patient, families and health care team
- Identifies and directs availability of resources to service identified needs of patients using clinical judgment and guiding MD to facilitate DC.
- Collaborates with and completes referrals to appropriate community agencies and other facilities and communicates with medical team
- Documents case management activity in a timely manner in the medical chart with team, patient/family and community resources.
- Works independently within the assigned area and is accountable for the development and maintenance of service therein. Is responsible for seeking consultation when needed and keeping supervisor apprised of patient/program developments. Work is reviewed through medical chart documentation, reports, conferences and feedback from other disciplines
- Provides case management coverage for complex discharge planning as assigned
- Maintains a stated level of quality while minimizing resource usage and decreasing length of stay and using nursing judgment to communicate with medical team and external stakeholders.
Minimum Requirements: - Graduate of an accredited school of nursing with a minimum of a Bachelor's Degree.
- 3-5 years discharge planning
- Active RN License in the District of Columbia, required.
- Case Management Certification within 1 -2 years of employment
- Cerner-Sorian a plus
- Case Management Certification is preferred
